Our Agency started with a focus on the specific needs of individuals involved in the justice system, making Axiom Care one of the most trusted resources for this population. We offer substantial programming to individuals specifically designed to build personal accountability, manage emotions, find pro-social alternatives and increase a sense of community. Axiom Care has developed treatment services and partnerships to become one of the leaders in helping clients find freedom from the life of probation, parole, and community supervision.

VERN JOHNSON
Founder

Mr. Vern Johnson started as a social worker and has devoted over 25 years of his life to health care; he consulted for 7 years to develop and implement a new business concept for 17 hospice & home care start ups.
He moved on to partner in several projects regionally, including two large facilities in Mesa, Arizona, as well as several hospice programs around the country. Vern served as CEO of Building Blocks Counseling and is on several committees; he has held various board positions for nonprofit organizations such as the National Hospice Organization, Arizona Health Care Association, 1n10 and Arizona Recovery Housing Association.
Today, Vern is involved in brokering projects in development, especially in behavioral health, nursing facilities and assisted livings. He brings extensive experience in the service provision of health care in hospice, home care, assisted living, behavioral health and skilled nursing facility businesses; as well as taking an active role in Axiom Care.



DUANE MANTEY
President – Owner

ViVRE Recovery Housing was Mr. Duane Mantey’s vision. He has worked tirelessly since 2010 to develop partnerships and support to build the ViVRE Program into the success it is today.
Duane’s passion is focused on improving the lives of those involved in the justice system, specializing in low level offender re-entry, and providing a safe and structured living program for otherwise homeless releases.
Duane is originally from Washington, but made Phoenix his home many years ago. He is involved in many social organizations including DACA and Toastmasters.
He can regularly be found presenting at community events on recovery related topics. Duane is currently president of Axiom Care and serves as president of the Arizona Recovery Housing Association.



JOHN WAKE, PHR
Chief Executive Officer

John Wake is Axiom Care’s Chief Executive Officer and has been with Axiom since 2020. Prior to joining Axiom, John was Director of Human Resources with a large multi-state recovery and treatment entity.
He has over ten years’ experience in substance abuse treatment and is passionate about improving the
effectiveness and accessibility of treatment in Arizona.
John’s strong human resource background comes into play in this role, as all aspects of interpersonal relations is the guiding force in his managerial style.
John received a Bachelor of Science in Biomedical Engineering from Arizona State University.
John is an Arizona native, an ASU grad, and lives in Tempe with his wife and son. He enjoys hiking,
travelling, and finding great new places to eat.



JIM ELDER
Chief Development Officer

Jim Elder is Axiom Care’s Chief Development Officer. Prior to joining Axiom, Jim was Director of Business Development and Client Outreach at a large behavioral health organization. There he was tapped to evaluate and lead the expansion efforts across the US.
Jim’s background prior to this role was heavily weighted in sales leadership and workforce development, including directorships in admissions and enrollment. He consistently contributes to the strategy of the company with his views on workforce and regulatory feasibility.
Jim is a Pacific Northwest native, who has spent nearly 30 years in sales and marketing in various organizations. He has four adult children, three of which are still in college, and his passion is to spend as much time with them as possible in outdoor activities. When not working or with his kids, Jim likes to explore Arizona on his Harley.



MARK C. ANDERSON, PHD
Chief Operations Officer

Mark Anderson is Axiom Care’s Chief Operating Officer. After obtaining his PhD in Psychology, Mark entered private practice as a clinical psychologist. While carrying caseloads of patients, he developed and executed training programs for pre-doctoral students during their internships. In addition to training programs, Mark also grew to roles as an instructor, researcher, program director, supervisor all
while performing clinical psychotherapy to groups and individuals.
Mark’s consistent leadership roles led him to join a behavioral health company as Chief Program Officer with P&L responsibility. This high-growth company experience and increasing levels of responsibility led him to his current role as Axiom’s Chief Operating Officer.
His professional designations include: Nevada Licensed Psychologist, New York Licensed Psychologist, and Arizona Licensed Psychologist.



JOHN POKORNEY
Chief Financial Officer

John Pokorney is Axiom Care’s CFO and has 30+ years of financial management across a variety of industries and company sizes. John brings experience in financial crisis and turnaround management, new business development, capital transactions, M&A, financial modeling/data analytics and managing companies through restructuring. His business philosophy is based upon communication, collaboration and contribution. His financial management philosophy is cash is king, connections are essential, and compliance are the table legs for all great accounting and finance departments.
