Patient Case Management
The purpose of our diverse Case Management team is to assist and support each of our clients with different aspects of their lives related to substance use. Our team is focused on helping individuals with homelessness, healthcare needs, nutrition assistance, employment, legal issues and other barriers that might impact their ability to successfully complete treatment and establish long term recovery from drug and alcohol addiction.
These services may include but are not limited to:
Connecting client to Primary Care, Medical Assisted Treatment, STI/HIV testing and treatment, and other community-based health organizations
Assistance with transportation
Coordinating clients with healthy, positive support systems
Assistance with Employment and Job Readiness
Developing healthy coping and life skills through coaching and mentoring
Identifying housing opportunities in the community
Neither treatment nor case management are one size fits all. Case Management services are individualized for each client, while services focus on specific needs identified during an assessment and developed within an individualized treatment plan. The case manager’s relationship with the client is established during the first week of admission and continues well throughout the aftercare stage of treatment.
Peer Support
Axiom Care provides an opportunity for individuals who have achieved a significant amount of recovery in their lives to assist other clients in our addiction treatment programs, through their journey of recovery by sharing their experience, strength and hope through mentoring. More information will come soon about our new and exciting Peer Support Program!
Support Groups
Our case managers at Axiom Care work closely with clients to identify their individual needs. We offer a wide variety of support groups such as financial, literacy, life skills, recovery based support groups such as AA/NA/ Smart Recovery, and many others to assist with developing skills that support a transition to independent living once treatment has successfully been completed.
HEA Plus
All of our Case Management staff are certified with the State of Arizona/ DES to complete and enroll all clients without insurance, or to assist with renewals for AHCCCS and Nutrition Assistance. Ensuring clients are connected increases health outcomes and promotes a healthy well-being within the individual while in treatment.
Employment Services
The Case Management Team works with clients to develop resumes based on previous employment history and experience, as well as helping to develop job building skills, assists with identifying felony friendly job leads in the community, and collaborates with partner agencies such as Dress for Success, OIC and Native Health to obtain clothing and other employment related items in order to be successful at securing employment.
