Axiom Care’s Set for Success Program is dedicated to supporting individuals with Serious Mental Illness (SMI) as they transition from the justice system to community living.
Our 90-day program provides comprehensive mental health support, without any cost for housing. While staying with us is not mandatory, we offer the convenience of bringing our services to each individual!
Our mission is to provide all-inclusive and compassionate support tailored to the unique needs of those with SMI, addressing both their mental health and their reintegration into society. Together, we will navigate the healthcare system, explore long-term placement options, and help them achieve their personal goals!
Program enrollment requirements are:
1. SMI determination
2. Transitioning from incarceration
3. Willingness to participate in programming
About the Set for Success Program
Our Set for Success Program takes a holistic approach to post-release support. It encompasses various components that are designed to facilitate a transition for individuals with SMI and set them up for long-term success. This includes:
Optional Free Housing
- Tailored facilities to meet the needs of justice-involved individuals with SMI
- A supportive environment of stability and community
Mental and Physical Health Support
- Psychiatric evaluations, medication management, and access to healthcare professionals
- Multidisciplinary approach to address complex mental health needs and co-occurring disorders for those with addiction
Counseling Services
- Individual therapy, group therapy, and behavioral interventions
- Development of coping skills, self-awareness, and addressing underlying issues
Assessment and Goal Setting
- Comprehensive assessment to identify strengths, challenges, and aspirations
- Collaborative goal-setting process with input from patients and their support network
Regular Progress Reviews
- Ongoing monitoring of progress toward goals
- Adjustments to care plans as needed for continued success

Setting You Up for Success
Beyond addressing immediate needs, our program also sets individuals up for long-term success. We focus on stable employment and permanent housing by offering support such as:
Work Force Development
- Programs to enhance job skills

- Partnerships with employers to facilitate job placement and retention
Housing Placement Assistance
- Guidance in finding suitable, stable housing
- Support with navigating housing applications and requirements
- Relationships with housing agencies to streamline the process of finding stable housing
Preventing Recidivism and Homelessness
One of our primary objectives is to break the cycle of recidivism (the tendency for a convicted criminal to re-offend) and homelessness among formerly incarcerated individuals with SMI. Our program focuses on:

Life Skills Development
- Assistance in managing finances, accessing transportation, and other essential life skills like cooking and cleaning

Frequently Asked Questions
How do you get referred or placed in the Set for Success Program?
SFS clients have referrals sent to Axiom by DOC’s medical provider, Naphcare. A referral must be sent while the client is still incarcerated.
Can I smoke cigarettes or vape nicotine while in the Set for Success Program?
The clients are able to smoke cigarettes or vape nicotine in designated areas only.
Do I have to live onsite to be enrolled in the Set for Success Program?
It is not a requirement for clients to live onsite with us. We do offer wrap around services to community based clients. These services include case management, individual counseling, and mental health groups.
Can I work/have a job while in the Set for Success Program?
All clients are eligible to work once they have met the program requirements. The requirements include compliance with program rules, participating and attending all scheduled clinical services, and providing negative UA’s.
What if I’m on probation, can I enroll in the Set for Success Program?
No, the Set for Success Program is exclusively for those on parole.
