Axiom Care is pleased to announce a new 60-bed transitional housing program designed to serve Veterans navigating challenges related to substance use disorders, mental health conditions, trauma, and homelessness.

Opening November 18, 2025 at 804 E. Jones Avenue in Phoenix, the program is located on the campus of Victory Place, a 200-apartment Veteran-focused affordable housing community.

The Veteran-centered housing and treatment model provides a safe, structured environment where Veterans can engage in intensive treatment for up to 90 days, gaining the stability and support needed to transition to long-term housing while continuing treatment.

The program offers a supportive living environment and access to outpatient services to ensure continuity of individualized care.

The Veteran’s Journey

Residents will have streamlined access to Axiom Care’s integrated health services including evidence-based clinical treatment, medical care, and psychiatric services, as well as workforce development training and Art Of Our Soul art therapy through nonprofit partner Axiom Community of Recovery.
